Additional Enrollment Forms (2021-2022)
Once you have finished your online enrollment, please review and complete the following forms.
These forms MUST be received by the office before your child begins Nursery School.
Completed forms can be mailed to Amie Bloom, C/O Temple Israel, 600 New Scotland Ave, Albany, NY 12208 or faxed to (518) 482-5762
Child in Care Medical Statement (OCFS-LDSS-4433) To be completed by a Licensed Physician, PA, or NP
In order to register your child, you must set up an account AND add your child to the account. Without your child listed on your account, you will receive an error message that states, “no one can be enrolled right now please contact your school administrator for more info”
