Enrollment Forms 2023-2024

Temple Israel Nursery School creates an inclusive and welcoming community for all preschoolers. We encourage participation from all regardless of religious background, race, ethnicity, gender, ability, sexual orientation, or gender identity. We are committed to being a diverse program that welcomes all children and families.

Children can enroll in Temple Israel Nursery School at any time throughout the school year as long as we have an opening in our classroom.  Registration for the following school year begins on February 1 for the upcoming school year and on March 1 for the summer camp program.  Our school offers a 10 month school program- September- mid June, and a 10 week summer camp program that begins in June and ends in August.

Once you have finished your online enrollment, please review and complete the following forms.

These forms MUST be received by the office before your child begins Nursery School.

• Medical Statement: Child in Care Medical Statement (OCFS-LDSS-4433) To be completed by a Licensed Physician, PA, or NP
